Typically it's at your 'big' u/s at about 20w. I found out loud and clear with I was having a boy with my first at 15w3d via u/s. We didn't expect to be able to see that early, but it was perfectly clear. People find out earlier if they do an amnio or a CVS. CVS is usually performed at about 11-12w and an amnio is usually around 16w.
For most moms they tell with an ultrasound at 20 weeks. That ultrasound also checks for a number of physical issues. For determining gender, this is not 100% accurate, but is very very good.
For older moms, or moms who have a genetic predisposition to certain disorders, some will chose a test called an amniocentesis (or amnio), in which a doctor will remove some amniotic fluid to test it for certain genetic abnormalities and will also tell you the babe's gender (XX or XY chromozone testing). This is usually done around 16 weeks. Because you're sampling baby's DNA, this should be very close to 100% accurate.
