Both work on the brain's perception of the pain not the site, though advil is also anti-inflammatory to the blood vessels in the site which may be helpful. On the other hand it may make it bleed more in some people.
Personally I think tylenol is nothing but liver-poison and has never done anything for pain worth mentioning. I would go for 8mg codeine phosphate which is OTC in Canada, Australia, the UK and most of the western world.
