My friends set out a cute baby-themed mat (?) with space for a photo of the baby (like people do for weddings) and everyone signed it.  Once we have a pic in there we will get it framed - it's really cute.

That was my idea as well.  There will be 10 or less people at my shower so we may not do it but I love the idea!

Originally I told my sister (hostess of my shower) that I wanted to do some sort of signature mat, so when the baby was born we could put the mat with all the signatures around a picture of the baby, frame it, and put it in the nursery.  She decided to do one better and got a scrapbook.  Each couple got their own page and wrote a little note to the baby or us and had a chance to decorate the page the way they wanted.  We had a photo printer so we were also able to put a photo of each couple at the shower into the album right away.  The scrapbook is such a cute keepsake, I love looking through it.  She managed to get all of the things that she needed for the project (scrapbook, paper, stickers, etc.) for $20 or $30 dollars from Michaels with a coupon.

For the ones I've thrown, we've done a picture frame or a hardback children's book. I love the children's book idea b/c everyone could right a little note to the baby. We usually use Goodnight Moon.

My SIL had everyone write on a cute piece of paper some 'mommy advice', I'm going to put those in the baby book

for my girlfriend, i printed out cute advice cards and put it at each place setting for all the women to fill out.  At the end, I tied the card up in a bow & gave them to her, she then mounted them in a section of her baby keepsake scrapbook.
