I need to write an email to my old boss asking her to mail me my 2008 tax form. I worked in a very small office (7) and although she was good to me while I was there, she really upset me right after I quit (when I had DD) and so I haven't talked to her in a year. I also never took DD into meet her like she wanted me to.
I need to draft a short email asking for the form. I don't want to be rude and just say "send me the form!". Any help would be appreciated! TIA!!

(PS: you might want to invite her to lunch or something... it is never a good idea to have a bitter employer if you decide to resume your career later. After all this will be the first person potential new employers will call).
