How to mix formula and breastmilk?

I have some bottles of breastmilk in the fridge. I want to add formula to the bottles so he is getting half/half to start. Does the formula have to be the same temp as the breastmilk to add?

Should I use the filtered water from the fridge, make the bottle with the formula, then pour in the breastmilk?

Since the BM is cold then I made my formula with hot water so when they were mixed it would be warm. I use my tap water since its fluoridated. My dentist said that since we use the tap then down the line we wil not have to give fluoride suppliments. When we are out we use the water from our Brita filter which does not take out the fluoride as most bottled water does.

When i first started supplementing with formula, i would buy the ready made Similac, it was just easier.  I didn't use powder until he was 100% FF.
