Anyone write a maternity leave proposal to their Boss?

Did it just include your due date, and expected amount of time that you will be out? -Or- Did it include more?

I just filled out my paperwork yesterday...I put down my plan to work until I deliever or am recommended by my doc to stop. I put my due date and then that I would take 6 weeks disability to recover from the pg and 6 weeks FMLA to bond with my child

I was approved for all 12 weeks :0)

I also included anticipated start of leave (e.g., 2 weeks prior to due date, until I go into labor, etc.).

Mine included when i would be gone (with estimated dates), coverage for those dates (using sick time, vacation, Short term disability, etc), proposed date of return. I also included daily, weekly and monthly job duties (I wanted them to be sure to cover everything so that things didnt fall apart and my boss didnt *really* know what I did everyday).

I was going to write one about working from home, but just talked to my boss instead.  We don't have to write proposals for maternity leave.
