When do people start hiring doulas/midwives? Do people do one over the other or both?

You can hire them pretty early. It depends on the demand in your area. You’ll pay the same either way because a doula really does their work in the 3rd trimester. 

I have a doula because I’m going to a hospital and having my doctor or another in the practice deliver the baby. 

If you want someone to provide prenatal care and also deliver the baby, you want a midwife. Midwifes are often nurses as well. Medical training.
