ivf with icsi

After 12 days of injections it was finally time for my retrieval yesterday.  They got 31 eggs! they said my husbands sperm was great as well.  They called me today and said they got 20 great eggs from it however my eggs are not fertilizing and they should have some indication of fertilization at this point.  So he is going to do icsi and I got really upset.  Has anyone had icsi be successful after natural fertilization hadnt happened? I will get a call tomorrow with an update on my embryos and if icsi worked or not.

  • Hi @kindrex05 - I'm so sorry to hear about the stress you are facing. Brutal. I personally had ICSI, though it was planned from the get-go. We were unsuccessful but the first time we had a difficult transfer so there wasn't much of a shot, and the second time it was a frozen transfer and well... I guess it just wasn't our time. 

    Obviously that isn't overly helpful to your question, but I wanted to check in and offer you my support regardless! Wow 20 great eggs is fantastic. Impressive! Are you upset because you don't like the idea of ICSI or that it is an afterthought and are worried about the result after all of your effort? To give you some stats - we only had 7 viable eggs (nothing compared to your 20), and all had ICSI and 4 were successfully fertilized.

    • Nov 15th 2016 - Started daily Lupron Injections for upcoming FET
    • Nov 22 - Baseline US/BW - Intralipid Infusion - Start Meds for FET with immune protocol
    • Dec 16th FET transfer of 3 embryos (1 - AA, 2 - BB)

  • @kindrex05 I really hope everything works out for you! I've been wondering if it were possible to switch to ICSI if they weren't fertilizing. Please keep us updated! Fingers crossed for you!!!

    Clomid 50 mg 3/15 (unmonitored) - BFN
    Clomid 50 mg + metformin 4/15 (unmonitored) - BFN 
    First RE appt. 5/15; Natural cycle 5/15 monitored with 2 mature follicles and Pregnyl Trigger (full dose) + prometrium - BFN
    6/15 HSG - clear tubes & normal uterus; great PCT test results
    TI - 100 mg Clomid + prometrium (AM & PM) + vaginal estrace (AM & PM) 6-7/15 (monitored) --> no additional response and thinned lining - BFN
    TI - Injectables (follistim + Gonal-F, Ganirelix, & 1/2 dose Pregnyl) + prometrium (AM & PM) 9/15 --> 3-7 mature follicles (3 definites and 4+ that could have matured due to trigger) @ O -->BFN + 5 large cysts
    BC for 2 weeks due to cysts
    TI - Injectables (Gonal-F, Ganirelix & full dose Pregnyl) + prometrium (AM & PM) 10/15 --> 1 mature follicle --> BFN 
    TI - Last attempt at injectables (Gonal-F, Ganirelix & 1/2 dose Pregnyl) + crinone (AM only) + vaginal estrace (AM & PM) 11/15 --> 3-4 mature follicles --> BFP!! 11/27/15 @ 13dpo (shockingly, actually waited until then to test)

    Beta #1 @ 16dpo (11/30/15) = 1,075
    Beta #2 @ 19dpo (12/3/15) = 3,150
    One baby: Saw heartbeat @ 5w5d (114 bpm; baby measuring 2.3mm)

  • I just went straight to ICSI. It's standard at my clinic. I think you'll have a great chance at that because of the way they clean and pick very specific sperm. And the fact that they place it right in the egg. Perhaps you have hard egg shells. Or perhaps the sperm sample just was my optimal. I hope it all works out for you. It did for me.

  • Hi ladies! I was upset bc I felt I didn't know what it really was and bc I had so many eggs and his sperm was great. I ended up getting 6 fertilized after icsi so it really did make a difference and I can see why some people are saying they had it planned from the start. I'm only on day 2 of my embryos developing and so far so good! I'll have a 5d transfer so they will keep me informed daily on the status and quality until Tuesday when I get my official transfer day and time!

  • Congrats! I'm so glad the switch to ICSI worked for you and helped your cycle. GL and fingers crossed for a healthy sticky bean for you!

    We will probably be transfer buddies. My ER was on Friday and I have my ET on Wednesday! Do you know if you will transfer 1 or 2? My DH and I are on the fence and want to decide based in egg quality... We are leaning towards 2 tho since this is our only try before the insurance is tapped.
  • @kindrex05 I also did ICSI from the get-go and it definitely worked out well.  Do you have unexplained infertility?  If so, I think the positive in your story is perhaps that your issue is fertilization, which now you have conquered!  Clearly many more steps but that is a big hurdle!  My transfer is 9/29 so we are also very close!  :)

    @HBamama2B we did the same thing for our fresh cycle earlier this summer - waited to see based on blast quality.  We had 9 top grade and 8 just one step down (1AA and 1AB? I don't remember how they categorize them and all clinics are different...they called us 'fertile myrtle', which was just too ironic to even respond!).  Anyways, the embryologist actually told us that with the high quality blast transfers success rate is very similar if transferring 1 or 2, just your chance of multiples increases to like 40%.  Therefore we went with one.  Clearly BFN though, so who knows.  For this FET we are transferring 2.... we know the risks and figure it really is out of our hands so deciding not to overthink it this round!  At the end of the day I defer to the specialists.  Hopefully it will be clear for you what to do on Wednesday!!

  • @mclorn @HBamama2B thank you! I just got my official call that my transfer is going to be Wednesday but definitely on the fence too about 1 or 2! It is definitely an uphill battle with so many decisions. I am also unexplained so yes @mclorn it actually made me feel better in a way like I had some sort of answer in all of this!! Happy transfer week to all!!

    So... there's a great chart on egg quality for chromosomes and age that I used.

    My math process: They say there's limited statistical difference, but that's bc they pool candidates. If you break it down individually, some women have much higher success and some lower with 1 vs 2. The reality is at my age of 34.5 its statistically assumed that ~30-40% of eggs are abnormal and won't fertilize, implant or will lead to miscarriage. So assuming some of that 30-40% are among the mature eggs that didn't fertilize, that means there is still a significant number ~20-30% that won't implant or won't grow and no way to know. In addition we have MFI with morph issues so some of that may increase implanting/growth. We elected not to do PGS since it's not shown to increase PR in our age range which leaves implanting 1 with ~30% chance of implantation failure or 2 with a 30% chance of implantation failure each.

    The SART stat I think is most telling is the % of live birth twins for my age range and diagnosis. It's devastating to lose any embryo period. But still having one growing? Idk. The decision just had to be personal rather than mandated. IMO

    For instance, this is also our last shot as we have tapped out our existing insurance so no follow up FET for us.... Which is why we are so on the fence. I think if we were doing FET next month it would be easier to say 1. Anyway sorry to dump! Just been wrestling my brain with all this!!!

  • I agree .. To be told not to stress, this is an agonizing decision to be made. It's all so much .. I really like my dr and I feel he will help me make the right decision

    "Fertile Myrtle" to a couple obviously struggling with infertility? Yikes! Either it was someone's first day on the job, or they forgot the morning coffee. Either way, I'm sure they were instantly embarrassed and won't be saying it again!

    August: DH started clomid
    November 2017: Started metformin for egg quality
    March 2018: Planning to do IVF + ICSI, antagonist with HGH, freeze at 2pn stage for 2018 FET
