Cancelled IVF and changed to IUI with stimulation drugs

I am new to this board, but I wanted to see if anyone had some advice or has been through my similar situation. We have been trying to conceive for almost 2 years. Tried Clomid for 3 cycles and 1 IUI through our regular OB. I was then referred to a fertility specialist and we just tried our first IVF treatment and it was cancelled (after 12 days of stimulation) due to a low response. We decided to do an IUI this cycle since I had 2 really good follicles on my left side (18mm and 21mm before I triggered). I have been diagnosed with DOR (diminished ovarian reserve). My AMH was 1.1 and FSH was 15.6.

Has anyone had success with IUI after cancelled IVF and has DOR? I feel like it was a good chance since I had the stimulation drugs to help my little eggs mature and I know my follicles were a good size. My Husband a good motility rate - 98% and 47mil (post wash).

Just curious if there is anyone out there with my similar situation....

  • hi..I don't have your exact situation but I am in a somewhat similar boat as you.  I also have DOR my amh was around 0.2 and my FSH was 14.2 at its highest.  I haven't tried IVF (yet) but we have done 2 IUI cycles and are about to start our 3rd.  I have heard of people with DOR getting pregnant through both IUI and IVF but I know with higher FSH responding can sometimes be a problem making IVF not the best option.

  • I have no experience with this because I'm waiting to have my first IUI soon but wanted to wish you good luck on your upcoming IUI.  Sounds like the stats are lining up well for you. I think it's smart of you to try for the IUI because of them.  FX for you!

  • I don't have this experience either, but I wanted to wish you good luck. It does sound like you have a pretty good shot with the IUI. I really hope it works for you!! Please keep us updated and let us know when your IUI is!!

    Clomid 50 mg 3/15 (unmonitored) - BFN
    Clomid 50 mg + metformin 4/15 (unmonitored) - BFN 
    First RE appt. 5/15; Natural cycle 5/15 monitored with 2 mature follicles and Pregnyl Trigger (full dose) + prometrium - BFN
    6/15 HSG - clear tubes & normal uterus; great PCT test results
    TI - 100 mg Clomid + prometrium (AM & PM) + vaginal estrace (AM & PM) 6-7/15 (monitored) --> no additional response and thinned lining - BFN
    TI - Injectables (follistim + Gonal-F, Ganirelix, & 1/2 dose Pregnyl) + prometrium (AM & PM) 9/15 --> 3-7 mature follicles (3 definites and 4+ that could have matured due to trigger) @ O -->BFN + 5 large cysts
    BC for 2 weeks due to cysts
    TI - Injectables (Gonal-F, Ganirelix & full dose Pregnyl) + prometrium (AM & PM) 10/15 --> 1 mature follicle --> BFN 
    TI - Last attempt at injectables (Gonal-F, Ganirelix & 1/2 dose Pregnyl) + crinone (AM only) + vaginal estrace (AM & PM) 11/15 --> 3-4 mature follicles --> BFP!! 11/27/15 @ 13dpo (shockingly, actually waited until then to test)

    Beta #1 @ 16dpo (11/30/15) = 1,075
    Beta #2 @ 19dpo (12/3/15) = 3,150
    One baby: Saw heartbeat @ 5w5d (114 bpm; baby measuring 2.3mm)

  • I'm curious about your stim cycle and what drugs you used. I'm also a little surprised that the treated you as straight DOR with an amh up there at 1.1. 

    No IUI success here - but wishing you luck!

  • Hey guys, thanks for the responses. I am currently in the tww( day 11 to be exact). I go back for my beta test on Thursday, but I jusy couldn't wait and tested today... It was negative. I can't help but feel so disappointed. Everything seemed to line up perfectly.

  • @riveridgional I though I had included my stimulation if. Not sure where it went. I was on the micro dose lupron protocol: Dexamethasone 1mg one a day Letrozole 2.5 mg one per day for 5 days Microdose lupron 10 units morning and night Gonal F 450 units at night Menopur 2 vials at night Day 6 of Stimson I added omnitrope each night Did all of this 12 days and only had 2 mature follicles on the left side with 2 additional ones smaller than 10mm and 2 on the right side smaller than 10mm. Anyone else had a low or poor response?
  • I have allot of experience with this.  I have DOR with AMH of .5.  We went into every cycle knowing that it would be converted to IUI if I had less than 4 follicles.  This happened to me four times over the course of my many treatments.  The last IUI was a success with two follicles and I am now 7 months pregnant.  With DOR its really just a matter of getting a good egg that cycle and there is no way of knowing that.  Just try to remain positive and don't give up hope.  
  • @jcgirl1101  Did you ever get a successful IVF (by successful I mean made it to egg retrieval?)  Or did you always have to convert to IUI? I have DOR and my husband thinks if we just keep trying IUI's we will eventually be successful and that IVF is a really big gamble because of how unsure we are of my response and my low AFC woudln'tmake it worth while.  Do you mind me asking how old you are? I am 32 (almost 33)

  • I went to retrieval 3 times.  First time had three 5 day blastocysts.   I transferred two frozen blasts and ended up with a blighted ovum.  The clinic I went to wanted to get a few more blastocysts before we did another transfer.  Two more retrievals and none of the embryos made it.  So i was told to consider donor eggs.  Before I did that I transferred by last on only embyyo and it ended up working.  I have a perfect 1 1/2 boy at home.   We started trying again last December and did a natural IVF that failed, regular IVF where I actually responded with almost 7 follicles growing.  Went to retrieval and they weren't able to retrieve anything.  Third cycle they change meds and I did not respond so converted to IUI which thankfully worked as they were encouraging me the donor egg route again.   I was 34 when i started and 38 now.  

    I think IVF has the best chances.  It takes a couple of cycles to find the drugs and dosages.  And each cycle you may respond differently.  But I did go into every cycle knowing that it may be converted to IUI which is better than being cancelled.  With DOR is really about the quality of the eggs and not quantity. Its a challenging diagnosis so I would check the stats at your clinic to see what percentage of patients they treat with DOR.   
  • thoughts on if I took a test today (12 days post IU) and its negative that I will get a positive blood test? I have cried all morning feeling like I am just a BAD EGG, literally. I am looking for hope, but also the realistic truth. When I asked my nurse today if I tested and it was negative should is still come in for the blood test on Thursday she said yes, they still want me to come in. Thoughts?
