Water leakage, or broken Terrified.

I'm 21w4d with twin girls and I was admitted to the hospital yesterday because my water was leaking or broken. They checked and both babies have good strong heartbeats. The US saw there was 7.3 and 7.1 AFI. So, I started thinking it was going to be ok. Couldn't sleep with the IV fluid and antibiotics going in my right hand and the blood pressure cuff on my left squeezing every hour. They gave me Ambien and I slept. But this morning the doctor called and said that the babies won't make it bc my water broke and they can't survive at 21 weeks. I asked about bed rest until 25 at least to protect the babies from losing more fluid but she said she has never seen babies survive that way. But I know @AMShark‌ is making it. So so terrified. I'm sorry if this a duplicate post and am only seeking compassion and support. Please delete if this thread bothers anyone.

I'm so sorry this is happening!  Big T&Ps your way.  Has the leaking water slowed down?  That does seem odd that your doctor is jumping to such a dire diagnosis...and to say she's never heard of babies surviving that way is ridiculous!  Is it that you have some unique diagnosis or circumstances that makes her say that?  Can you talk to another doctor?

Omg. I'm so sorry to hear this. Please know that I am thinking of you and praying fiercely for you and your girls. I think that you should push for what you think is best. Is there a different doctor that you can consult? Or a higher nicu that you can be transferred to like @AMShark‌ is doing? Please keep is updated.

I'm so sorry you are going through this. If that was my doctor, I would be questioning him/her big time. Have they checked the fluid levels again today? If the leaking has improved and the babies are still maintaining decent fluid levels then I don't see why bed rest would not be the first thought. I believe Amshark had to wait until 23 weeks to check in for bed rest, but at least she was given a proactive plan. It seems as if your dr is just writing you off and that is not ok. Definitely ask to get other opinions.

I am so sorry you are going through this! I will tell you one thing you need to be at the closest level 3/4 nicu in your area that takes babies at 23 weeks and you need to be seen by the top MFM team to formulate a plan. People in your position have carried babies to viability and beyond. It's never over until it's over and don't you dare give up! If you live in Massachussetts I will give you names to call.

I didn't get where I am today because I listened to any doctors who told me I would never have children so you need to be a strong super woman and tell these people what to do!

(SAIF) I'm with @luvboson. You have to be your own advocate. If you aren't in labor and you still have some fluid and heartbeats, you aren't out. I appreciate that your doc isn't saying you are fine, but they are wrong that it is over.

Good luck, this isn't going to be easy. I'm really sorry you are in this place. Get as much fluid as you can and rest. You are in my prayers.

Oh no @starlightgator‌ huge hugs! I agree with others that I would want a second or third opinion and options laid out. If babies are doing fine now then hopefully they can come up with a plan to keep that going for a few more weeks. I'm so sorry you are going through this.

@gsancho‌ Thank you so much. Yes, I haven't seen much leaking since I arrived. They tried to get a sample with a paper that would turn blue, but it was inconclusive. Showed a little blue mostly green. My MFM came today and had much brighter outlook. Still very concerned and insisted I stay in hospital (using a nasty bedpan). But, great fears of infection and fever. If that happens, it's the end.

@Justone13‌ Wow! Thank you. I remember you from a long time ago. My MFM seems to have a better (still scary) report than the other doctor in my OB's practice who I first heard from. I'm told that this hospital has the best NICU in the state.

@bears1011‌ Thank you. I don't think there has been more fluid but it's hard to tell. So, they're treating it as if my water broke and a lot of waiting. I'm counting on my MFM's opinion now.

@Ducky719‌ Thank you. No signs of infection and praying no fever. Because doc said that's the end. No leakage as far as I can tell today. It's not my OB but his partner at the practice and I think she is also the attending. But, my MFM is very good and he is here too.

Hugs. Definitely trust your MFM. This type of situation is exactly what they are trained for. Sounds like your MFM is being understandably cautious, but also much more encouraging and that's what you need right now. Always ask to see or speak with your MFM whenever you're concerned, and certainly before decisions are made (with the obvious exception of an emergency situation). Sending more hugs.

I'm lifting you and your baby girls in prayer right now, that the leaking will slow/stop and that infection stays at bay. Also praying that your doctors would be wise with your care and supportive - to be realistic while giving you and your babies a chance. Try to stay calm and optimistic, and advocate for you and the babies! We're here for support. Keep us updated!

Glad your MFM gave you a better and more realistic expectation.  Granted, you don't want them to tell you everything is fine if it isn't, but the earlier doctor was giving up too quickly!

Maybe consider a transfer to JMH. The MFMs are always there and if anyone has seen craziness it's those folks there. Academic center...different can of worms. Maybe talk to someone there?? (305) 243-4960. Best of luck!!!!

@starlightgator - I'm so sorry I've been off line all morning.  (I was having issues with the wifi the last time I logged on, and I just hadn't tried again).

I had a very similar experience.  My OB gave us a very bleak outlook, as he had never seen PPROM at 20w make it to birth, however, he also admitted that he did not have a great deal of experience with PPROM that early.  All of his talk, though, was geared towards loss or severe disabilities and birth defects.  My MFM on the other hand decided to go ahead with treatment as if I had a viable pregnancy.  That meant antibiotics, and monitoring.  They *did* send me home for bed rest until I actually was closer to viablility, but I was admitted to a hospital with a higher level NICU at 23w2d, and given steroids at 24w, and I'm now at 27w3d, and doing well overall.

I'm so sorry I didn't get a message to you sooner.  I may have been able to relieve some of your stress by letting you know my experience with my MFM compared to my OB.

You're doing everything you can right now.  If you have no signs of infection now, hopefully, the antibiotics will keep you in the clear.

Any word on the possible dilation?  I had a roommate with PPROM who also had a cerclage.  Is that an option for your situation?

Ok...so going into doctor mode. The fluid levels look great from what you say, and the nitrazine test was inconclusive. Why are they so convinced you ruptured membranes? There are ways to confirm that if the rest of the testing is inconclusive. Are
you still feeling like you're leaking? If the fluid levels stay stable, I can't imagine you actually broke your water. I'm assuming they ran vaginal cultures. Any results? It's not done often, but when testing is super inconclusive, they can inject a blue dye or sterile milk into the amniotic sac (like a reverse amnio) and then you wear a tampon and walk around in it. If there is no leakage, you are done. Of course, the needle now increases your risk of actually breaking your water, but it seems like they haven't really determined you are truly ruptured. 7cm of fluid for each twin at this gestational age is ok. AFI isn't measured the same in twins as in singletons. Grill the MFMs. Get them to explain. Don't get written off. Keep us posted....

You are the kindest group of online women I've ever "met." Thank you. Here's the update:
Last night was rough but thankfully DH was here with me. Had a horrible painful experience with the IV. It started throbbing worse and worse and took the nurse forever to come in. Turns out it was bent in my vein. She took it out but I was hysterical crying by then. Convinced her to ask my MFM if I could just keep it out and take oral antibiotics. Approved! Had another set of monitors early this morning and again an hour ago showing both sweet heart beats and felt kicking for the first time afterward.

MFM came back this morning. Happy to hear no more fluid loss. Wants transvaginal US on Tuesday to check fluid and cervix. Wants to move me to a better room. Agreed I can go home Tuesday to stay on bed rest and then back at 24 weeks if US looks good.

Never had fluid tested bc they said it's too early to show up on those strips. Mentioned the blue dye procedure you talked about but also agreed too risky bc it's like an amnio.

So, sitting here and waiting, talking to the babies, singing to them (hope outsiders don't think I'm a loon), and praying.
