I am so sorry to ask a drug related question...I know that you must get sick of them.  But if I may, I would appreciate it=)

Have you seen alot of people start off with a initial 10 mg dosage of Aricept?  Not doing the adjustment period?

I have.  I don't dispense the 5mg very much at all.  From what I've read it's really not been determined to be very effective, unless you're getting someone at the very early stages.  The jury is still out on the overall efficacy of the drug.  But for some, it's better than nothing.
