1st Trimester

My DR says she doesn't know what's going on...

Hey girls, could use some advice / opinions.  I should be 6w4d right now, but this pregnancy has been anything but expected...

Had an ultrasound yesterday that showed only a gestational sac that measured 5w1d. No yolk sac, no fetal pole and nothing in the fallopian tubes. Now, it did take me 27 days for a follicle to grow after femara, so I was hoping I maybe ovulated late even after ovidrel and it implanted late, which is why my measurements would seem off.

But then there's my betas...I dont know what to make of this, they were doubling fine to start, then slowed WAY down and are now speeding up again... and my RE keeps going back and forth with "you have an ectopic, you need to rush to the ER with any signs of pain" and "you're having a miscarriage, your betas are going to go down and you'll start bleeding".

She scheduled me for another ultrasound on Tuesday and repeat beta, but given that she keeps changing her mind about miscarriage vs ectopic, I don't know if I should just go get a second opinion.

Advice? Words of wisdom?

  • 6w4d is based on when I took the Ovidrel trigger...and for a little while, my doctor was saying that it was measuring smaller because maybe it took a day or two longer to ovulate and then maybe longer to implant than normal.
    I completely agree...I have cried for so many days in a row because of their flip flopping...I honestly just want closure. I feel like I'm losing my mind going back and forth...

  • I think you should try for a second opinion as your doctor seems very unsure of things.
  • I don't have any advice but I just wanted to say good luck!  And I think there is no harm in getting a second opinion.

  • Thanks for the advice / well wishes girls!
    I talked to my regular OBGYN who said I'm not a "clear picture" and that it could be either a blighted ovum, regular intrauterine pregnancy or, less likely, ecopic...so still no real answer.
    I go in today back to the reproductive endocrinologist for another beta and an ultrasound. Hoping and praying it's just a stubborn baby who's taking its sweet time developing and not something terrible...

  • Got the call with my results this afternoon and had an ultrasound this morning. It's not looking good...my heart hurts from this. I just keep thinking how unfair it is that my husband and I, who love each other more than anything, can't make a baby.

    I don't know what to do...I don't know if I should just get it over with and do it this week, or wait a couple of weeks for a miracle.

  • So sorry you're going through this. I had a blighted ovum last year and its a major mind fuck. I miscarried naturally the day after I found out, but I only found out at 12w 5d. The sac had stopped growing at 6w 6d, which shows how long your body can hang on to the pregnancy. It's up to you if you potentially want to wait that long. A D&C would get you back to square one quicker, but you need to know if you're comfortable with the procedure first.
    Give yourself time to heal and I hope you get the support you need.

  • I don't have any good advice other than waiting. I am going through the same thing but they haven't checked my betas at all. They said it was pointless. I should be around 6 wks give or take. That website mentioned is really good. (Misdiagnosed miscarriage.com.) I scheduled an appt with a different dr that is tomorrow. I'm going to ask them to check. I hope we both have good outcomes. Prayers for ya.
  • I went through a missed miscarriage a year ago. Tomorrow is my 1 year anniversary of when I found out. I went in supposed to be 11 weeks 4 days and was only measuring about 7 weeks so my body hung onto it for quite awhile. I opted for a d&c and wanted to get it over with. The whole thing was a very difficult experience. I'm sorry you are going through it now.
    The good news is that you are likely to have a healthy pregnancy in your future. Many people that experience a loss go on to have healthy babies. I am currently 22 weeks pregnant and so far so good this time. Hugs

    I'm sorry to hear, that's never easy. Your body should take care of everything on its own this early so D&C isn't always necessary but obviously go with your doctor on that. But, there was a girl a few weeks ago who sounded just like this and she is ended up being fine. Apparently she had twins, one died so that's why it looked bad. She nearly did the D&C but waited a week and that's when they saw the living fetus. Hard call but after reading her story I'd wait a bit and continue to be monitored, especially since some people ovulate later than they thought

    I actually tried to get in to see another RE for a second opinion but no one seemed to have appointments until JUNE.
    I scheduled another ultrasound next Wednesday and am hoping and praying they see something...
    Thank you for that website...I thought I was going crazy holding out hope. It's really encouraging to see that this doesn't always turn out badly. ...and that my tilted uterus may be somehow 'hiding' the baby.
    I'm still not expecting a great outcome, but at least I'm not bawling my eyes out like  I was for the last 24 hours.

  • Hey girls, quick update...
    Started bleeding a few hours ago. Ran to Er because of the ectopic precautions my Dr gave me.
    Just got hooked up to iv fluids because my heart rate is so high and blood pressure low.
    Waiting on ultrasound and labs.
    Please send some prayers this way....no matter how strange this pregnancy has been, I don't think one could prepare themselves for this.
    Btw, great day for both my ob and reproductive endocrinologist to go out of town...

  • I wouldn't wait for a miracle, but it's okay if you want to wait a few weeks to give your body the chance to miscarry naturally.

    I'm sorry for your loss.

    And FYI one pregnancy loss doesn't mean that you and your husband can't have a baby. It just means that this one wasn't it.
    I think the OP is referring to her battle with infertility that is posted in her siggy. Adding this to that battle is a bit unbearable.

    OP, I'm sorry you are going through this and I hope you get some answers soon.

  • So, quick update.

    They gave me IV fluids in ER because my heart rate was so high and blood pressure so low...they took labs and my blood count was good, just white cells up a little, so decided I was just very dehydrated.

    My beta today was 5736 (from 3093 2 days ago)...).

    They did an ultrasound and it showed a gestational sac measuring 5w4d and they may have seen a yolk sac, but are pretty doubtful (saw something that resembled a little circle, but really weren't sure). Also, saw a subchorionic hemorrhage, so I get to be on 'light duty' for the next little while (no exercise, no lifting, and bedrest as much as I can)

    They said that it could be a blighted ovum still, or a pregnancy with my dates being off (5 weeks 4 days instead of 7 weeks 3 days, which I don't understand because I know exactly when I ovulated...24-36 hours after I took the ovidrel).

    Anyways, unfortunately, I got no definite answers today either. They told me the bleeding should stop on its own, eventually. Repeating HCG on Saturday and ultrasound next week...it's like the nightmare that never ends...I honestly just want an answer at this point of if it will be a blighted ovum or not, so I can move past this.

    Thank you girls for your good thoughts! I really appreciate it. Today was honestly one of the scarier days that I've had in a while.

  • I'm so sorry :(.

    From my personal experience, your betas and u/s stats don't sound promising :(. I have had two D&Cs, but was much further along and it wasn't blighted ovums. If I were you, I would probably give my body a little time to do it naturally, and then ask for something like misopristol to induce miscarriage. While I appreciate the choice to do a D&C, it is surgery and not something to take lightly.

    At the end of the day, you need to wait until you are 100% confident in your choice. If you need a second opinion - get one. If you need to wait a few weeks for peace of mind - wait. For each of my miscarriages I was 100% sure the pregnancy was indeed over and never second guessed myself.

    I think molar is the type of pregnancy that yields higher betas.

    OP, your beta seems to line up correctly with the ultrasound estimates. This is all very confusing. I'm just thinking, if you ovulated 36 hours later and then the embie implanted two days after that, you could easily be off by several days.

    I actually go back on Wednesday too. I really feel like I'm having a miscarriage now though...started bleeding heavier today / passing things that look like tissue / having bad cramps. I truly hope that you have better luck than I did...I'm devastated

  • Well, I'm finally going to have my answer.

    After the bleeding last week, they repeated my beta on Saturday and it had gone from 5731 to 5081 in 2 days...went to the OB filling in for my regular OB yesterday and he did an ultrasound that showed an empty GS. Since it's pretty much been this way for the past few weeks, he thought that the wisest thing to do would be a D&C to decrease risk of infection.

    Honestly, I'm happy I finally have an answer since I was losing my mind going back and forth, but I'm devastated that this is how it ended and terrified about the D&C tomorrow.

    04/13: Clomid, IUI BFP --> MC at 6w1d
    05/13: Femara 2.5mg, IUI --> BFN  , 08/13: Femara 2.5mg --> BFN
    03/14: Femara 5mg, IUI --> 1 follicle @ 27d --> BFP! EDD 12/02/14--> blighted ovum, missed MC 6w6d --> D&C
    4/23: D&C...starting over again, with a little part of my heart broken off
    5/31: Femara 7.5mg --> cancelled cycle, no follies
    7/14: Femara 5mg + brevelle + menopur + IUI --> converted to IVF, ER 7/28 --> ET cancelled due to severe OHSS.
    9/20/14:  Frozen Embryo Transfer --> BFP--> EDD 6/6/15 --> MC at 5w3d 
     
    10/16/14: Frozen Embryo Transfer --> BFN
    2/6/15: Frozen Embryo Transfer --> BFP --> MC at 5w4d
    3/20/16: PGS-tested Frozen Embryo Transfer --> BFP, Living Child born 12/1/15
    6/6/17: Fresh IVF Cycle --> Severe OHSS, 5 PGS-tested embryos frozen
    2/23/18: PGS-tested FET --> BFN
    3/30/18: Cancelled cycle due to lining 4.2mm
    6/21/18: PGS-tested FET --> BFN
