Preterm Labor and Delivery Birth Story

I briefly mentioned our twin girl delivery in the second trimester check-in, and here is the longer version if you are interested....

It has been hard to write about my birth experience.  I tried to write an email to out of town friends after I delivered, and it took me two days and many tears.  I was even more of a mess on the phone.  Now that we are two weeks out, my daughters have stabilized somewhat, and so have my hormones.

On April 10th, I called my OB office because I was having a little more cramping and "Braxton Hicks" than usual.  She saw me in the office and did an FFN and called me 1 cm dilated. I was 25.3 weeks. She sent me across the street to the hospital's triage to be ruled out for preterm labor and get betamethasone.  The triage doctor called me 2cm and decided to admit me and give me betamethasone, indocin, and mag for 24hrs.  By the time I got to the labor room, I was 3 cm, and DH had arrived from work.  I was contracting a lot, but not in any pain.  The FFN was positive, but it didn't really matter at that point because my labor was progressing so quickly.  My OB recommended that I formally transfer care to MFM, so we did.  The NICU doctors came to our room to give us an idea about what to expect from preemies this age.  Of course we didn't sleep much that night, and by morning, I was still 3-4 cm.  After the mag at 5pm, I went to the antepartum floor, and I had all but stopped contracting. 
I stayed the same until Thursday morning, when I had a few more contractions, but my cervix didn't change...but then came Thursday night, and the contractions started to feel different.  I was moved back to labor and delivery.  Through the early AM, I dilated from 5 to 6 while DH slept on the couch.  I knew in my heart to let him sleep as much as possible because this was going to be the day.  I was 26 weeks, and by the time daylight came, the contractions had started to last longer and even were slightly uncomfortable. I was 7 and then 8cm.  My water never broke.  I knew I would get a c-section because baby B was breech and so pre-term. 

Laura and Katherine were born under calm circumstances, rather than an emergency, at 11:10am and 11:11am on Friday, March 14, 2014.  I saw them as they rolled out to the nicu, and then not again until 3pm.  They were born at 1lb 12oz and 1lb 13oz.  We were thankful for 2 girls, and we waited until we saw them to name them.  They were stabilized in the nicu, and have been doing pretty well, considering, so far.  My c-section went very smoothly.  They did a lower transverse incision on my skin, but they cut the uterus vertically (or classical incision), so I will not ever have a vaginal delivery.  Just a bit ironic because I dilated to 8cm without much pain.

Laura and Katherine are both still intubated, but the pediatrician is very pleased with their progress so far.  They do tend to desat often (mostly before mealtimes!).  They are taking fortified 24cal breastmilk through ng tube, voiding and stooling, and they have both passed their birthweight!  We are such proud, yet helpless parents.

    5 rounds of clomid, 3 IUIs, and multiple non-medicated cycles- BFFN                                                      
    IVF with ICSI in August 2013 brought us our babies.  ER-9R, 7M, 4F w/ICSI  ET of 2-Grade 2 blasts. 
    + HPT at 6dp5dt. #1 (8dp5dt)- 105    #2 (13dp5dt)- 510  #3 (15dp5dt)- 960  #4 (17dp5dt)- 1889
    Dx shortened cervix, PTL, and preeclampsia during pregnancy.  Lots of medications and 13 weeks of bedrest, babies were born healthy at 34w4d!

    Me: 42. DH: 46.

    1st Pregnancy: MC, 11/19/00.

    2nd Pregnancy: DS born 04/10/06.

    3rd Pregnancy: CP, 03/11.

    4th Pregnancy: MMC, D&C 11/30/11, Genetic testing revealed Trisomy 4.

    5th Pregnancy: Ectopic, 2 doses of Methotrexate unsuccessful, surgery 4/10/12, right tube removed.

    Tried Letrozole January 2013-July 2013 (including 2 IUIs), all BFN.  After 2 1/2 years of trying for child #2, decided to "give up" after July cycle, based on AMA.

    August 16, 2013:  BFP our first month of "not trying!"  Still in shock.  Beta #1 (14dpo): 183.  Beta #2 (17dpo):  611.  Ultrasound 8/30/13: baby measured 6 weeks, 1 day, heart rate of 118 bpm! 
    Ultrasound 9/13/13:  8 weeks, heart rate of 176!
    Baby is due 4/26/14
