I know cloth can be bigger. Should I be stocking up on leggings and t shirts? If so where do you buy them? Also, can you still use onesies and other outfits? Do you have to size up?
A was in newborn sized clothes still, and has always been close to her age size. We've only ever had to size up for her height, not her diaper. Since you're having twins you can probably expect them to be smaller than my average/tall sized singleton.

My kids fall on the small side (assuming your twins most likely will too). My 1st was in disposables and her age/size was always a bit big. My 2nd was in cloth and age/size was pretty spot on. In fact, I thought the CDs actually helped keep pants up on his small frame. The 3rd is also small (6lbs), and is wearing NB clothes just fine.
For the summer months I do end up dressing them in t-shirts (Carter's had the best size selection for under 12mths) and baby legs if needed at home. I'd still put on shorts or pants if we went out.
I've never had a problem with fit of clothes... Like ITK said, we've only had issues with height. I thank God we use cloth because I think it holds her pants up - tall and skinny doesn't work for baby clothes!
my little guy, 5lbs9oz about the size of an average twin, was in NB clothes for 2.5- 3 months with cloth diapers. It was July so we hung out in onsies and no pants or one piece outfits a lot.
We never sized up. My daughter had very chunky thighs and pants with "real" waist bands were a problem for us, anyway, so we did a lot of knit pants and leggings. You can definitely still use onesies. You can get extenders for onesies, too.

We've never sized up on anything due to diapers. DD was a very average size newborn (7 lb 5 oz) and wore newborn size clothing (not 0-3 months) for nearly a month. That included onesies and zipper sleepers. With twins, you'll likely get even more use out of newborn size.
The only NB sized stuff we didn't get much use from (2weeks maybe) was the tight pants. Looser pants, onesies, everything else she was in NB 4-6 weeks. She was 6#13oz at birth.

Other than those, we always went up a size one month before the actually size.
And my little man fit into newborn for about 2 to 3 weeks. And he was 7.5 pounds at birth.
