Trouble TTC

New and question about HSG

I'm new to the board and I'm probably going to have an HSG this week (if I can get the scheduling worked out).  I was wondering how painful the HSG will be.  Is it like the cramping pain that you get with an IUI?  Also, I trying to decide if I should take the whole day off of work. 
Me: 29, DH: 31
TTC since May 2012, married since 7-08-06
Me: irregular cycles, possible endo
DH: bilateral varicoceles (surgically corrected on 12-13-13)
Treatment: 3 cycles of Clomid alone, 2 cycles with Clomid and IUI
HSG Feb. 14:  all clear
1st appointment with RE 3-20-14
2nd appt with RE: normal postcoital test, u/s showed a large follicle the day after I got a positive OPK, most likely problem is endo :(
2nd HSG May 14: My RE used a dye that is supposed to help calm down inflammation from endo

At 12 mos I decided to look for another doc. 02/2013 consult with new doc....3 days later testing began...u/s, all good, HSG: all good, labs: all good, ovulating, etc. Started on Pregnitude. 04/2013 failed attempt for SA, 05/2013 failed attempt SA. 12/2013 SA: 3% morph, abnormal functionality. DH started Pycnogenol. RE consult 01/15/2014.

  • mines upcoming too, ive heard most women can go back to work no issues, but some were glad they took the day bc they got to go home and sleep :-D im super nervous about it.

    I've had two and both were so easy and no pain!! 

    The first was done in a hospital and I was terrified. I felt a slight cramping (like a period cramp) when he put the catheter or whatever in and I was bracing for the pain of the dye and didn't realize he had already put it in until he told me to turn to each side (to direct the dye). Cramping only lasted a couple minutes).

    The second time was at my RE's office where they are, of course, much more specialized and do these all the time and it was even better (though I didn't realize that was really possible). No pain. Super quick.

    I did take advil before each one but am not sure to what degree that changed anything. I didn't work either day but easily could have. I've only recently had the one IUI, but the cramping was similar.

    Me: 32  DH: 33  Married: March 2004

    July 2006: started TTC
    2008: HSG (normal), couple rounds of clomid through gyno
    2008 - 2010: dragging my feet out of fear and procrastination
    October 2010: first consultation with RE, dx PCOS and fibroids (DH slightly low count/motility)
    Oct. 2010 - Dec. 2012:  In DENIAL! avoided the issue because I was scared of surgery
    January 2013: returned to RE, fibroids grew significantly
    February 2013: second HSG, fibroids pushed on tubes which blocked them somewhat
    March 2013: MRI to determine what type of surgery may be necessary

    July 29, 2013: fibroids (5) removed via robotic laparascopy
    August 2013 - Nov 2013 : benched due to recent surgery

    IUI #1, Dec. 24, 2013, BFN 
    IUI #2, Jan. 25, 2014, BFN
    IUI #3, Feb. 25, 2014  BFN
    IUI #4 canceled due to lack of response to letrozole
    IUI #4.1 April 28, 2014, BFN

    May 16, 2014: wtf consult, start prepping for IVF in June and add injects for one last IUI in the meantime
    IUI #5 started letrozole and bravelle but canceled after HSG led to new diagnosis

    May 21, 2014: third HSG, tubes blocked, one at the beginning, one hydrosalpinx??
    June 11, 2014: consult, approved to move on to IVF because the hydro is not completely blocked therefore allowing fluid to move through slowly rather than backwards
    IVF #1 August 8, 2014 - 3dt of 2 embryos, BFN
    September 17, 2014 - 4th HSG, the right tube is very patent (open!!) dye went straight through this time. Weird!
    October 2, 2014 - started metformin treatment
    November 14, 2014 - blood work, brought A1C down from 5.8 to 5.5

    November 26, 2014 - RE finally back from vaca and reviewed my chart, no more IVFs for rest of calendar year
    December 1, 2014 - Right after Thanksgiving, I called a new clinic and got in right away! Plan for IVF
    December 17, 2014 - ER! 29 retrieved (!!), 16 mature, all 16 fertilized (ICSI)

    IVF #2 December 20, 2014 - 3dt of 3 embryos, BFN

    We are done with treatment unsuccessfully. :(

    It's different for everyone. It all depends on what they find. I will say tat mine was painful due to blockages & I was still able to go straight to work from it. Even if it's painful, the procedure is very quick & the cramping doesn't last long. 

    ETA: Taking some pain reliever about 45 minutes ahead of it helps a lot.

       Me: IR-PCOS, elevated DHEAs, low progesterone, weak ovulation  DH: low volume, low T
    SHG 5/10/13: both tubes blocked; HSG 6/28 = Left tube cleared! Right blocked.
    BFP#1 7/20/13 EDD 3/30/14, m/c 8/19/13, D&E 8/21/13, Chromosomal results = normal, female
    Lap & hysteroscopy scheduled for 10/31, right tube cleared, no endo found! ...Happy Halloween!
    Cycle 14: Clomid (50)+ IUI = BFP! EDD 9/16/14~ Rowan Elizabeth born sleeping at 17w4d on 4/12/14 due to IC.

  • Mine was painful, especially when they shot the dye...it caught me completely off Guard. But the good news that it was quick, like 30 seconds maybe. Then I was fine. I worked from home that afternoon, and I found that I was really exhausted, probably emotionally exhausted more than anything and I had mild cramping the rest of the day. I did not have any blockages. Best of luck! I hope yours is easy!

    As others have mentioned, it varies a lot from person to person. 

    Mine was kind of comically miserable, but it was due to a some unique circumstances: 
    The doctor was not my usual one (mine was on vacation) and was male (I'm used to a female doc); in the room there was a burnt-out light-bulb in an area where they needed light (so the nurse actually had to bring a flashlight); and apparently my cervix was at a difficult angle (the doctor described it as "going around a corner").  All of this, along with being in a cold room, and being anxious about the results, made me tense up - which made the procedure more difficult for them and more painful for me.  As others mentioned, the worst pain (if you have any at all) is when they shoot the dye and it does not last long.  I was a bit sore/crampy and bleeding for several days, but it wasn't bad and it is less common for that to happen.

    As I mentioned, this was an unusual set of circumstances, and the procedure took a lot longer than normal.  While I was getting dressed, I even overheard the nurse talking to another member of the hospital staff about how bad she felt about how terrible it went!

    Even so, I went straight to work from the hospital, as I originally planned.  If I knew that it was going to happen the way that it did, I probably would have taken the day off to recover, but it was okay.

  • I had no pain it just felt awkward and uncomfortable. It was so quick. The anticipation is worse than the test. You can do it! They show you the results immediately after and it is actually quite beautiful. I had minor aching but I went to work. It sort of felt like the pain you get after doing crunches, but lower. GL!

  • I had more discomfort with the HSG than with the IUI. It's only when they put the catheter in, that I felt discomfort, almost pain. Afterwards, I was completely fine and went about my normal activities. It doesn't last too long

  • My HSG was extremely painful. But, I also have a blocked tube from surgical scarring.  I have heard that it is more painful if you have a blockage.  I hope yours is perfect and pain free.

  • Thanks for the input!  I was able to get it scheduled at 1pm, so I'll just take the rest of the day off.  Thankfully, I don't have any reason to think there will be any blockages.  The only issue they can find with me is that I have irregular cycles.  My husband had bilateral varicoceles.  He had those fixed on December 13th.  It takes 3 months for sperm quality to improve, so we're waiting a few months to try another IUI.

  • Mine was easy. I had minor cramping but nothing terrible. I went to the gym after and did my usual 5 mile run. No issues at all.
    BFP: 5/10/14 after 14 cycles of trying!
    EDD: 1/21/15
    5/12 1st beta: 28
    5/14 2nd beta: 87.5
    5/23: 3rd beta: 4000.6 + saw gestational sac and yolk sac
    5/30: 4th beta: 17,852 + saw fetal pole and heard heartbeat! 127 BPM!
  • A friend of mine had a painful SHG so I was concerned about my HSG. I took 4 ibuprofen before hand and cramped slightly during the procedure (more than period cramping) but it was completely tolerable. I could have gone back to work, but I just didn't want to!

    OB/GYN: DH SA (Sept. 2011): Normal -- Round 1 (11/27/11): 50mg Clomid = BFN -- Round 2 (12/27/11): 50mg Clomid = BFN

    Hormone specialist: CD 21 b/w (May 2012): Normal -- Thyroid: slightly low

    RE: CD 2 b/w (Oct 2012): Normal -- HSG (Oct 2012) - normal (for the most part), Retroverted/Retroflexed Uterus. Otherwise normal shape and no tubal blockages.
    **Unexplained Infertility**

    Second opinion (12/16/13):
    DH: Excellent SA
    Me: Slightly elevated AMH, slightly low FSH, low thyroid, extremely low DHEA,
    Jan 2, 2014: Clomid cycle (100mg), 2 great follies at 28mm and 31mm (no cysts so far), great lining and fantastic timing = BFN
    Jan 30, 2014: Clomid cycle (100mg) + .5mg Dexamethason + 81mg Aspirin = BFN

    2015
    Testosterone level was 20Estradiol & Progesterone also low. Hormone pellets for 1 year + Progesterone. Numbers looked great while on pellets. After pellets, Testosterone down to 5! Taking a break from all the bloodwork & hormones.

    2016
    Looking at possible Th1 & Th2 elevations. Requesting laparoscopy.
    Currently taking Pregnitude, "Female Balance" herbs, DHEA.

  • It is crazy how different the experience is for different people.  I was completely worried about mine.  It didn't help that I could hear the woman before me begging the doctor to stop because it hurt so bad :(  To make me feel better the doctor decided to tell me how he was in the room when his wife had hers and she was cursing everyone out the whole time (thanks doc).  I did not have any blockages, but it was pretty damn painful.  I bit my lip and breathed slowly and it was over after about 60 seconds.  I have heard 1- the pain level is directly related to the skill level of the doctor, and i've heard 2- it hurts much more if they use a balloon catheter.  I have no idea if either are true though.  I had slight cramping the rest of the day like an IUI, but tylenol and a hot bath knocked that right out!  Good Luck!!

  • I had some cramping during the procedure, and mild cramping on and off after, but nothing major.

    I did have a lot of discharge after, so wearing a pad was helpful. I went back to work after.

  • My HSG was not terrible, but I have SEVERE menstrual cramps (in bed for a day) so I'm used to really, really bad cramping.  They did use the balloon catheter for mine and that was the absolute worst part when they blew that balloon up.  I was already cramping from the balloon that when they did the dye it didn't hurt anymore than before.  That being said, as soon as they pulled the balloon out all the pain was instantly gone. 
    Good luck with your HSG and hopefully it is pain and problem free:)  
  • Hello...I had an HSG is November and did not have any pain associated with the procedure, I also didn't have any blockages. I did not feel anything when the catheter was inserted and felt a little bit of cramping when the dye was administered. I have a tilted uterus so I had to move onto my left side so the dye could proceed up my uterus, but that was my only abnormal issue. It was really cool to see the results in real time on the monitor as well.

    I took 800 mg of ibuprofen and did work from home after the procedure. If I needed to go back to work I would have been able to since there was no pain afterwards.

    3 day ultrasound 17 follicles
    HSG: 11/13/13- tubes open
    DH SA: SUPER sperm (145 million, 84% motility, 22% morphology)
    All infection disease and immunity blood tests NORMAL

    FIRST IUI May 2014: 100 mg Clomid days 5-9, third ultrasound CD 13 revealed four follicles 27, 24 and 20 and 13mm. Trigger shot May 28 with IUI May 29 and 30 (fingers crossed) - BFN started spotting 11DPO :(

    IUI #2 B2B 6/23 and 6/24, three mature eggs and 130 million sperm! FX this is the month!  BFN 7/8

    3rd cycle benched due to cysts - TI with OPK tests -  BFN

    3rd mediated cycle: Clomid+Trigger+TI  (three follies left side, one on right as usual....what the hell right ovary get it together and produce some damn follicles!) 7DPO progesterone level 43 with NO suppositories YAY for a natural strong ovulation. Beta canceled started spotting 13DPO - Third time is not a CHARM! 14 day cycle WTF! Everything looks normal - RE wants to start injectables next cycle so this cycle I'm benched

    Officially benched until March....but still plan on trying the baking soda douche due to my excessive and thick CM

  • Mine was painful...I cannot lie but I think I was able to work through the pain because the doctor told me my tube was open. It like a very intense uterine cramp...it is over very soon but you do need to try to breathe thru it. My doctor tried to let the dye run out of the tube to see the lining of a fibroid I have on the outside of the uterus and I had to tell him I can't take anymore.
    Me: +35 DH: +35
    TTC: Since January 2013 
    DX: PCOS. Severe Endometriosis, Unicornuate Uterus w/only left tube and left ovary, Pedunculated fibroid (on the outside of uterus) and Anovulation. All conditions diagnosed 8/13
    TX: Metformin
    DH DX: MFI - low morphology, low motility
    Ultrasound shows both kidneys in spite of UU. 
    HSG showed clear tube on the left side. 
    Lap Surgery performed 1/9/14 to remove fibroid and endo (Stage 3)
    • IUI# 1 June 2014 started 100 mg of Clomid - 7dpiui Progesterone: 13: BFN
    • IUI#2  July 2014 started 100 mg of Clomid - 7dpiui Progesterone: 5.75: BFN
    • Natural Cycle - so shocked to be in 2WW - 7dpo Progesterone: 15.5: BFN
    • Working with new RE starting injectables in late August.
    • IUI #3 August 2014  w/ Menopur: BFN
    • Finally ovulating on my own!!

  • So...I tried to have my HSG today and they wouldn't do it.  The nurse at my doctor's office didn't tell me that you need to either abstain from sex or use condoms for 7-10 days before the HSG.  Today was day 8 of my cycle and I figured there was no way I could be pregnant on day 8.  I didn't think it would make a difference if we had sex earlier in the week (which we had).  I talked to the nurse manager at my doctor's office and she said the preregistration person should have told me.  The preregistration call usually comes about 2 days before the procedure so I'm not sure how that would have been helpful.

    I was prepared to do the HSG today.  I took ibuprofen, had pads with me, and my mom was even there for moral support.  I'm going to be so mad if we can't get the scheduling worked out next month.  I work at a hospital and I need to have the HSG done there if at all possible for insurance coverage reasons.  Also, I'd really rather not have to go somewhere unfamiliar.  The HSG isn't offered every weekday at my hospital b/c there's not always an interventional radiologist on site.

  • I'm sorry that it didn't work out! :(  I wasn't told to abstain, either.  They just included a pregnancy test in my bloodwork about 6 days before the HSG.

    I hope that they are able to reschedule you soon in a way they you can prepare again as you did.  I know the waiting can suck, hang in there!

  • How frustrating. Hopefully it will work next cycle.
    10/2013 - Initial consult. Started levothyroxine - tsh 2.92
    HSG- showed graying in one spot/Saline Sono- showed 5mm polyp
    DH SA -  abnormal -  36%motility on both SA's (count and morph ok on SA1, a little off SA2)
    1/10/14 - Hysteroscopy to remove 5mm polyp
    1/12/14- Medicated Cycle 1: 50mg Clomid + Ovidrel + TI = BFN
    2/15/14 - Medicated Cycle 2: 100mg Clomid + Ovidrel + TI = BFN
    3/18/14 - IUI Cycle 1: 150mg Clomid + Ovidrel + IUI = BFN
    Upped Levothyroxine to 50mg - tested at 2.62
    4/17/14 - IUI Cycle 2: 150mg Clomid + Ovidrel + IUI = BFN
    Break!
    6/21/14: IUI Cycle 3: 150mg Clomid + Ovidrel + IUI
