From previa to accretia

So my ob did an MRI and it turns out I have placenta accretia, not just previa...
BC it can result in severe hemoraging I may have to get a hysterectomy which wouldn't be the best, but then this was going to be our last one. I am really more concerned about a) bleeding out and dying, leaving dh with 2 kids and b) the doctor said id need to be delivered ( most likely) at 34 weeks and nicu scares me..,especially after meeting which that damn neonatal specialist while I was in the hospital.
I'm also transferring my care to a high risk ob that my regular ob recommended.
Oh well, at least they let me come home from the hospital today anyway.
Has anyone here had this or know someone who did who had a decent outcome? I understand this is rare... (Sorry for the typos, I'm limited to phone bumping right now)

  • I've only seen one in real life. She was a full term vag delivery of 2nd child. She delivered a little after mid night and the OB wanted to go home and back to bed. She had an undiagnosed accreta and the OB was tugging on her cord to deliver the placenta quicker. By the time they realized something was wrong, they couldn't prep the OR in time. If the OB hadn't been in such a rush they might have discovered the problem without causing the fatal hemorrhage when it dettached. I was part of the RRT/code team that responded from the ER to L&D. You are very lucky indeed that they were able to discover this. Properly managed in an OR I think you will be in good hands.

  • This has been a fear of mine ever since I researched previa. I go in two weeks to see where my placenta is fingers crossed it has moved. I can't imagine how scared you must be right now but know that the healthcare system today is amazing. Thoughts and prayers are with you.

    I am very glad they know...you know my OB's office kept telling me - well the high risk OB we are consulting with wants to check, but don't worry, we have no reason to think it's this way!

    I am thinking that when I tell the high risk OB that he can go ahead and take my uterus and I have no objection, and he goes IN knowing that, it will really simplify things for him. It's the hemoraging that I'm really worried about. I seriously wish I'd updated my life insurance before this pregnancy. I know that sounds morbid but it's my big fear right now...with the intermittent impeding fear that the baby won't get here safely.

     

    ETA: @mummyofsix I do need to learn more about the risk of the hysterectomy. From what I can tell about reading about it, having the hysterectomy may be the best way to handle the risk of hemoraging, though. But I haven't had much of a chance to research anything yet. Thanks so much for the referral to the babycenter board.

  • @mustardseed2007 I shared the story because things didn't have to turn out like they did.if the OB hadn't been in a rush to go back to bed, they would have noticed the placenta not detaching, and would have taken her to the OR and discovered the problem. In the OR I really don't think you'll have a problem. They will be expecting hemorrhage and will be prepared for such. They'll have the level 1 rapid infuser set up, any auto transfusion equipment, and such.

  • What i meant in my case was that I didn't want a precautionary hysterectomy in my case because they didn't have a definitive diagnosis. My MFM wanted to do a 34 week c-hysterectomy "just in case". A c-hyst is risky, but it pales in significance to accreta and if I had your definite diagnosis I wouldn't be arguing with them. Sorry for not explaining that better, I didn't mean to judge your decision or make you worry more. xxx

    Hey no worries! Fwiw what they really told me was that they wouldn't know for SURE until they opened me up at delivery that a hysterectomy would be necessary, I just don't feel very lucky lately!
