Crawling and leaks?

We've been having more issues with leaks out the top of the front of DD's dipes, and I'm pretty sure it's because she is spending a lot more time tummy down and crawling. She's also started peeing a lot more. The front part of the insert is always soaked, and the back is usually pretty dry. Would a tri-folded prefold under a cover fix this? Or should invest in some fitteds?

Various brands of pockets. And I've tried microfiber, hemp, bamboo, and cotton inserts, and different combinations of those.

We had a lot of leaks around the legs when she changed positions. I changed to prefolds and changed more often.

I'd try to put as much of the insert to the front and middle as possible.
