WTF appointment for IVF #3

I had my WTF appointment today for my IVF back in July. While
I get a lot of eggs during IVF I don't get very many mature eggs ( last cycle 21 eggs 6 mature 4 fertilized 2 ok eggs transferred. My DR tested my AMH and it came back at 5. She said it's good, but when I looked up online it said it's actually not, any ideas? She also thinks that our only chance of conceiving is with donor eggs, I'm very open to this since we have already discussed it after the last failed cycle. She made a recommendation for the reproductive science center of New England. They have an egg bank so I sent a request in to get info. So yea that's where we are at. Still no clear answers.

DX: Infertile 6/11 Me: LPD, all other blood work clear.
DH: low sperm motility,low count.
Met with RE start clomid + TI + IUI 5/12= IUI cancelled due to positive Cystic Fibrosis carrier test. DH tested Neg. for Cystic Fibrosis,

6/12 Clomid + TI +IUI=BFN 8/10 IVF consult.
IVF #1 Started BCP 9/2 Lupron 10/11 stims 10/27 ER 11/6!! 5 eggs retreived only 1 mature. FERT report shows none made it.Cancelled cycle.

IVF #2 BCP 11/18 Lupron 12/14

Stims start 12/29 ER 1/8 ET 1/11 put in 1 grade B and 1 grade C. Beta: 1/20 = BFN

IVF# 3 7/13 BFP ended in chemical pregnancy

9/15 diagnosed with PCOS and insulin resistance gearing up for IVF# 4 Dec/ Jan

  • Hey jenn- so, your AMH is 5? Not .5? Have you been diagnosed with PCOS? I'm no RE, but isn't this a consideration with such a high AMH and a large number of eggs, but immature? What protocol did you do for ivf#3? Have you tried antagon or EPP? Have you tried the HGH injections during ivf? Have you tried all the supplements recommended (dhea, coq10, myo inositol , etc)? I can't recall if you switched clinics or have they all been with the same RE?

    10/11: after 2 years, saw a RE, FSH 5.4
    11/11: BFP! (surprise after thyroid & normal hsg),
    12/11: missed m/c after 7 week u/s, 1/12: D&C
    6/12 IUI#1-IUI #3: clomid = BFP!, C/P
    IVF #1(10/12) FSH 5.4, AFC: 16 long Lupron, 5R/5M/4F, all 4 made it to 5dt, 1 blast/1-8 cell transferred=BFN
    IVF #2(12/12)AFC 21, MD lupron, 4R/4M/3F, 5dt of 1 blast and 2-8cell. BFN.
    IVF#3(4/13) Natural start antagon protocol, 12R,11F. one PGS normal at day 6 transfer. BFN.
    IVF#4 (11/13) C.CRM (ODW.U normal 8/13 Still no Diagnosis) EPP/antagonist. ER 13R/7M/6F. Only 1 made it to freeze. Abnormal. Looking into options of DE, Fresh vs frozen.
    10/14 new local RE to look into what's next. CD3 FSH 4.7, AMH 0.9. Met with DE agencies and exploring options for feb/march 2015.
    Surprise natural bfp (4 days before donor is signed). Beta #1 at 9dpo: 51.8, 2nd beta: 195 (25 hours doubling) @11dpo. 3rd beta (12/15): 516 (35 hrs doubling) 4th beta(12/17): 895 (58 hours doubling) 5th beta(12/19): 2120. U/S at 5w0d(12/22): one gestational sac with yolk sac. U/S #2 (6w0d)12/29. One little bean measuring 6w0d with HR 124. 3rd u/s(1/4)7w0d: baby measuring 7w2d. HR 134. 3/30: A/S at MFM went great except for low lying placenta. Verifi results are normal! Team Blue! Please send any positive thoughts our way! EDD:8/24/2015

  • Hi J&D! She just said said the AMH was 5 which looked "good" I've never switched protocols always been on long Lupron with 150iu of Menopur and 150 iu of follistim or gonal-f. I am on coq-10 but nothing else. We have a second opinion coming up so hopefully a fresh set of eyes can find something. I just can't see giving up yet when we haven't tried anything different really.

    Jenn- I am a patient at RSC New England, currently undergoing a donor egg cycle.

    We are using fresh donor eggs, but considered using their frozen egg bank option.

    I know you still have a 2nd opinion to explore, but if you have any questions about RSC or their fresh or frozen donor egg options, PM me. I still have all of my paperwork/info and details and I'm living through the experience now.

    DE/DS IVF#1: (10/13) - 2 day 3 embies transferred-BFFN

    FET of 2 day 6 blasts: (12/13) - c/p

    DE/DS IVF #2: (4/14) - 1 day 5 blast transferred...BFFN...again.

    FET 5/14: 1 day 5 hatching blast transferred...another BFFN

    Repeat SHG 6/14-normal / Endometrial Receptivity Array biopsy 7/14-Receptive Uterus

    New RE, additional testing reveals elevated NK Cells

    FET of 1 day 5 blast (RE recommends transferring 1 due to elevated NK cells) with lovenox, steroids & intralipids in October

    I agree. I don't know all of your info, but it sure doesn't seem like a lost cause! It seems like you could just try something different and see how it goes. Different meds, different protocol, different supplements, etc.. I don't know if you are interested, but let me know if you want the list of supplements ccrm recommends and I'll get it to you. I still wonder why your AMH is so high, from what I read about it, that could mean you'd do better with a shorter protocol, like antagon or even MD lupron. Anyway, keep us posted and best of luck

    I'm feeling the same way as @J&D2007 , I'm surprised that your RE went right to DE.  It seems like you're able to produce eggs of your own, you just need to find a way to get them to mature.  I would get the second opinion and maybe there's just something your RE isn't considering.  But it must feel good to have also started your DE research, it is always refreshing to keep moving forward.  Good Luck!!

  • I agree with the others - get a second (or third) opinion before moving onto DE.   Unless you are comfortable with that route right now - I think you have a chance with your own eggs.   Changing clinics and labs is sometimes what makes the difference.

  • I don't know where you live but my dr used to work at RCs now she opened her own practice I would consult with her before going there. Also I would set up a phone consult with CCRM to get their opinion.

  • I agree with everyone else. Has your Dr said anything about switching protocals or just that you need DE? There are so many cocktails out there that if one doesn't work I think it's worth a shot to try something else particularly since you obviously have eggs. I hope that you're 2nd opinion appt goes well and gives you the answers you deserve!

  • Best of luck to you in the future. I agree that a second opinion is warranted. I had a problem with immature eggs, too. Do you think you were triggered too early? Several changes in my protocol helped me get the maximum number of mature eggs. We did EPP, HGH, & CoQ10. I hope you find answers.

  • Thanks for all the replies ladies! I feel more confident about using my eggs again. I do think it's odd my Dr never once suggested changing protocols. She just kept me on long Lupron and said I stimmed longer but after going through paperwork it doesn't seem as if I really did. We will be getting a second opinion from RSC, I feel confident working with them and seeing if we can still use my eggs, DE or not we will still work with them I think.

    Hi, sweetie -- just saw this post.  If you'll recall I have a similar issue where my eggs just refuse to mature.  I have moved on to DE.  BUT I think I'm significantly older than you.  And where you have gotten as much as a handful of mature eggs, I never got more than one.  If I were you, I would definitely go to a different, highly respected clinic or two or three and get second opinions about using your own eggs.  I have a feeling someone will be able to do something for you.

    If you have to go to DE, it will truly be okay, and you can get plenty of info about that on these boards, but there's plenty of time to think about that later.  To the extent you can afford it and tolerate it, it makes sense to feel you've exhausted reasonable OE options first.  That way you can proceed with no regrets.  

    Keep us posted!  I'm really hoping you can overcome this issue.  It sucks, I know.  There doesn't seem to be clear guidance on what to do from anyone.
