Did your Pedi provide an immunization card?

I know my mom always had this little record with all of the immunizations and the dates.  I needed a copy of it when I went to college!  Did your Pedi give you this or is this something I need to get on my own?

he has a "health passport" that they gave us at the hospital.  it has the dates & shots in it.

We got one at the hospital and then the pedi gave us another at his first appt

Oops, we got one at the hospital.  I guess I'm supposed to be taking it to the doctor's appointments and having him sign off on it.  Dropped the Mom Ball on that one!

I just ask my pedi to print one out for me before I go - daycare likes to keep a copy in their files, so that reminds me to get one!

Got it at the hospital, but in mass confusion of taking everything home, misplaced it, the pedi gave us a new one, we take it to all appointments.
