Moving forward? I need your advice, please please!

I'm coming to the end of my 2ww of our 3rd IUI (home BFNs). Our RE said after 3 unsuccessful IUI attempts, that we could move to IVF. We have a WTF apt already scheduled for is Friday pending a neg beta Thursday. This past IUI was my DH's worst sample yet, even lower an the minimum number they like to have to do the IUI but know my insurance won't approve IVF until 3 IUIs have been done so they did it anyway but said the odds arent great. The nurse said they were all mostly "sleeping". He has had borderline issues with motility and morphology all along.  The 2nd IUI produced our 3rd m/c you can see the rest of our history in my siggy. I'm very nervous moving onto IVF, as its a big upgrade in our approach to try to start a family. However, I want to do whatever is right for us to have a healthy baby on our own. We had 3 m/c, one chemical and 2 were chromosomal abnormalities (not permanent ones that reoccur but freak mistakes). What do you girls think? I'm trying to get some questions in line for our RE Friday and have a thought of what our decision will be, because she said it will be ours and gave us the consent form (16pages!?!) to read over to understand all it entails and to create questions. What are your thoughts after hearing this and seeing our struggles in our siggy? I really appreciate ANY input!

  • I didn't see a dx in your siggy, so I'm assuming mfi. If that's the case, it will be very difficult to conceive naturally, and it seems you are already at the right place. I'm surprised your nurse at least didn't go over the major items in the consent forms, actually. I never had a true iui option due to pcos, and if your insurance covers it, why not give it a try?
    On 10/23/13 Baby Sophie and Baby Gabriel born at 21+5 weeks. They grew wings and flew away from us. May God bless them always. We love you beans!

  • I did 3 iui's prior to IVF (not due to insurance, but due to hopefully having only minimal ART assistance).  When that didn't work, we wanted the best chance possible to have a baby.  Now that we had 1 failed IVF, they did more testing to find out i have immune issues.  Has your RE done a RPL panel on you since you've had the 3 losses?  I would do that prior to the IVF cycle to ensure your best chance.  

    The consent forms freaked me out too.  Signing those induced a panic attack but it's all stuff I knew I needed to sign, it just became "final" to me then and I got through it, so will you!   

     

     

    Me (37) DH (39); PCOS changed to Unexplained, changed to DOR in 2012 (finally a correct diagnosis!); 
    Started TTC 2009 with RE after 6 months.  
    Clomid + Trigger x2; 
    IUI + Femara x1,
    IUI + Follistim x2;
    IVF #1 (MDL) February 2013- BFN.
    IVF #2 (antagonist) May 2013, First BFP of my life. 
    Identical twin miracle BOYS (!!) headed our way- due date is technically 2/4/14 but c section is scheduled for 1/7!

    yes, we had the whole RPL completed and nothing was found. Only thing hurting us is MFI, my cycles are so regular every 28 days and I ovulate normally. Thank you!

  • We also moved to IVF after 3 IUIs. I know both my H and I just felt it was the right option for us, didn't feel we had a high enough likelihood of success with continuing IUIs. 
    Receiving our consent packet was pretty intimidating- it was similar in length. I just took plenty of time to read over it- leaving and coming back to it if I needed a break. I'm not sure how your appointment is set up but our nurse told me to read it over before coming in and then she went through it section by section with us.

  • I am sorry for your losses.  We don't have infertility coverage under my insurance unfortunately.  We skipped the IUI because I was at high risk for overstimulating and we also wanted to spend our money with the best percentage of success.  IVF is overwhelming.  It will however probably give you your best chance though with IVF and ICSI using a needle to inseminate your eggs.  You are young which is great and definitely working for you :)  GL on Thursday.  Hopefully, you'll get a BFP and not have to worry about IVF.

    Married June 2011 Dx: anovulatory due to prolactinoma (1.5cmx 1.5cm in 2006)
    April 2012: MRI- questionable cyst 8mg by 12mm in pituitary ; referred to RE by Ob-gyn after amenorrhea x 4+ months, provera ineffective, low estrogen level
    cycle 1: May 2012, clomid 50mg; cycle cancelled, thin lining, no response
    cycle 2: June 2012, femara 5mg; cycle cancelled, no response
    cycle 3: July 2012, femara 7.5mg; cycle cancelled (largest folli on Cd12 & cd 16: 11, lining 4)
    Repeat MRI July 25,2012- Cyst unchanged, likely old infarction.
    Aug. 8: met with RE, move to injectables if HSG and SA normal
    Aug. 23: HSG all clear; DH- perfect
    Switched to a new RE in early Sept. IVF here we come
    Genetic testing reveals: Fragile X- Intermediate risk/grey zone.
    IVF#1: ER 11/30: 14 retrieved, 10 mature, 7 fertilized. E2 prior to trigger 5200. Decision made to freeze 6 embryos and transfer later due to OHSS.
    IVF#1.2/FET #1: estrogen injects start 1/18, FET delayed to try to thicken lining. lining 5.4mm. FET- 2/18- transferred one 3AA expanded blast. BFP 5dp5dt. Beta #1- 2/27 9dp5dt- 102.27 beta #2 3/1: 147 :(, beta #3 3/4: 268 told to stop meds and let nature take its course. requested 4th beta (3/6); Beta hell for 2 months.

    FET # 2: endometrial scratch 5/6, added estrace vaginally and ASA to delestrogen and PIO. June 10- transferred one 4AA hatched embryo, BFP 4dp6dt, Beta 1 (9dp6dt) 187, Beta 2 (11dp6dt): 412; Beta 3 (15dp6dt ): 4452. U/S #1- one beautiful embryo with a heartbeat of 133 at 7 weeks.  8 weeks: heartbeat 156. EDD 2/26/2014: born 2-3-14 induced due to Pre-E and HELLP at 36w5d.

    12/1: met with RE to discuss trying for #2

    I'd look into an SCSA test for your DH. It's a different kind of SA, where they test the DNA integrity of his sperm.

    I've had a number of losses too and unsuccessful IUIs. We are still unexplained but always had "light" MFI - with things on the border of being normal. Finally we had Mister D do this test in Dec and discovered that almost 50% of his sperm were junk (you need less than 30% to be junk to have an IUI work). Through the advice our of RE, he was able to make some lifestyle changes and alter his result.

    Our original RE clinic poo-pooed the test but C.C.RM advised us to do it. I think it played a huge part in our different result in our most recent IVF.

    Another option is to get another opinion from a different clinic. I'm not saying your clinic is wrong or bad, but sometimes other clinics uncover other issues and/or can confirm that both clinics agree on the same plan moving forward. It can be very reassuring.

    Moving on to IVF.

    IVF #1 April 2012 = BFN, IVF #2 June 2012 = BFP. U/S 7/23 = saw heartbeat but measuring behind. Follow up U/S on 7/30 - no heartbeat. D&C 8/2. Trisomy 12. IVF #3 Oct 2012 = Chemical Pregnancy

    Phone consult with CCRM on 12/12/12 - ODWU 1/4/13 - both tubes clear(!) - AFC 24, AMH 3.2, FSH 9.6, LH 5.4, E2 25. DH has high frag rate but improved!
    IVF #4 March 2013 CCRM. EP protocol w/ Menopur, Gonal-F & Dexamethasone. ER 3/29 & IMSI, PICSI. 43R 13M 10F 6blasts bio'd. CCS testing reveals 3 normals!!!
    FET 5/31/13 of 1 4AA blast - thawed and expanded. 4dp5dt BFP.
    Beta 9dp5dt = 181, 11dp5dt = 427. 1st u/s showed a healthy heartbeat! EDD 2/16/14

    After 4 years of hoping and heartbreak, our sweet little bean was born on 2/19/14
    We are so in love with her.
