Engorgement?

Can it be in a form of just pain. I missed a pumping session yesterday. So when I got home I pumped for a longer time, thinking that would help since I missed a session. Well I woke up about 5 hours later in extreme pain. The pain starts in my up back and goes around to my breast. It lasted an hour and I took a pain killer and it didn't even do anything for the pain. I even pumped and that didn't help and I eventually threw up because the pain was so bad. This is the third time this has happened but this time was the worst it had been. I don't pump at night and never have, its just something I have never done and I still have enough supply to meet LOs demands. 

my breast doesn't ever fill full but they do feel heavy if I wait to long to pump. I'm a size "m" so I'm not sure if that makes a difference or not. 

Does it sound like I am just in engorged or could something else be wrong?

My guess is clogged duct. I've been there. Feel for hard spots

I don't have a plugged duct since I don't have any hard spots or any redness anywhere.
