Di/Di Twin Birth Story (37 weeks)

I was put on modified bedrest/procardia for irritable uterus /shortening cervix at 28 weeks. BR was successful as my cervix lengthened over the next few weeks. However there was some concern about a growth discordance with baby A measuring smaller.

Fast forward to 35 weeks where I am taken off br/procardia. At 36 weeks, mfm asks me not to go past 38 weeks because baby A's belly is only measuring 5th percentile. My cervix was already a 2-3 cm with 60% effacement, so OB suggests an amniotomy inducement at 37w4d.

The day before my inducement, I start having contractions at 3 am. They didn't really get into a good pattern, but OB wanted me to come in earlier since it was twins and my labor w/ DD was only 10 hours.

We were checked in triage at 6am and I was already a 6. U/s showed both babies were still head down (baby A since 28 weeks, B since 35 weeks). The OB was pumped for me to have a vaginal delivery (and was even willing to do a breech extraction with B before she flipped), but she did ask me to get an epidural in case of an emergency c-section (my last birth was med-free).

At 7 I got my epidural. I was already at 8-9 cm, so they wheeled me into the OR (just as a precaution). I was not feeling contractions/pressure/urge to push, so I had to rely on the nurses/OB watching the cx monitor. I was still able to push very effectively, remembering the technique from DD's birth.

Baby A was born at 7:53 am and Baby B followed at 7:57 am. She had the cord wrapped once around her neck, but pushing with an epidural was actually more controlled for me, so I had no problem stopping to allow for its removal. I also only required 2 stitches compared to the nasty labia tear from DD's birth.

I wanted to write my birth story to give hope to others who are dealing with a size discordance between A and B. A growth u/s 5 days before delivery had baby A at 5 lbs 12 oz and B at 7 lbs. My OB had zero qualms about doing a vaginal delivery even though most OBs require A to be bigger than B (or the one piece of peer-reviewed literature I found said it was safe up to 25% size dif).

Actual sizes were A 5 lbs 15 oz and B 7 lbs 1 oz. Good luck to all the MoMs-to-be!

  • Congrats and thank you for sharing. I'm having Di-Di and my baby B is slightly bigger but I'm hoping to have a vaginal birth also.

    Delivered by c-section at 32w0d 8/15/2014 due to preeclampsia/HELLP syndrome
    Baby Boy 4lbs 1oz, 17 inches
    Baby Girl 3lbs 5oz, 16 inches

    Congrats! My baby A was 5lb 10oz and B was 7lb. The size difference was not caught during any of my growth scans, even one week prior. I had a fairly uncomplicated vaginal delivery. A's cord was wrapped around his neck but they quickly removed it... I could tell right away that something was "wrong" when I started pushing for B. He was bigger and his shoulder got caught, but they reached up and helped him out. He was bruised but fine. He came just 13 min later. :)

    Married August 2012 -- Me (40 yrs old) My husband (45 yrs. old) 

    RE referral from ObGYN in late Jan. 2013 -- Testing Confirms Unexplained IF 

    IUI #"s1 - 3 (Femara + IUI) April - June 2013 - all BFN

      Decided to do IVF... Began stimming on September 10th!  Egg Retrieval 9/20/13.  19 eggs: 6 to mature to freeze after 6 fertilized with ICSI

     3 morula stage embryos transferred on 9/25/13.....Beta #1 on 10/4/13/ = 7.5  Beta #2 on 10/6/13 =  negative

      IVF#2 - Lupron plan  (set to begin on 10/28/13)  (Planning on thawing and fertilizing our 6 eggs with ICSI as well as all of the mature eggs they retrieve with this fresh cycle.)

     Late October 2013 - ectopic pregnancy - methotrexate injection....benched 3 months

    CD1 - 12/5/13...Lupron set to start on 12/26/13; CD1 - 1/2/14....Baseline 1/3/14....begin follistim tonight and continue with lupron.  (Got my calendar today!  IVF#2 is a go!)

    ER = 1/14/14  (11 eggs....9 were mature. 9 fertilized.  Of the 6 mature eggs we thawed, only 4 fertilized.  At this time, we have 13 eggs fertilized and growing.  Waiting for news about a 3-day or 5-day transfer.)

    ET = 1/19/14  (5dt).....3 blastocysts transferred!   (All nine embryos didn't make it to freeze.  =(  )

    Beta #1 = 1/28/14   (2 weeks after ER)  BFP! 209 (9dp5dt)   EDD 10/7/14

    Beta #2 - 1/30/14  (11 dp5dt) 302

    Beta #3 - 2/3/14  (15 dp5dt)  1222 

    Ultrasound set for 2/17/14......TWINS!        Twin A - HR 124 bpm; Twin B - HR 126 bpm

    Final Ultrasound with RE 3/10/14 - Twin A - HR 176 bpm; Twin B - HR 177 bpm.

  • Congrats! My baby B was much larger and I had a vaginal birth too! Baby A was 5lbs with a tiny head (~20th percentile) and Baby B was 6lbs 9oz with a large head (90th percentile). They were 1 hour apart and all was good!
